|Eligibility||All full-time employees working 72+ hours per pay period||All full-time employees working 72+ hours per pay period. Coverage can be purchased on yourself, your spouse and/or your dependent children. |Coverage||1x your annual base pay or a minimum of $40,000 for NKCH employees and a minimum of $20,000 for Meritas employees||Employee: You may purchase coverage for yourself in increments of 1, 2, 3, 4, or 5 times your annual salary up to a maximum of $500,000, with a guaranteed issue of $200,000.|
Spouse: You may purchase coverage for your spouse in $5,000 increments to a maximum of $150,000, but not to exceed 100% of the employee’s benefit amount. There is a $50,000 guaranteed issue.
Dependent child(ren): You may purchase coverage for your children of either $5,000 or $10,000. All amounts are guaranteed issue.
